Job Title: Home Health Medical Coder Reports To: Regional Director of Home Health Location: Remote / Hybrid / Carmel Employment Type: Full-Time

Position Summary

The Home Health Medical Coder is responsible for reviewing clinical documentation and assigning accurate ICD-10 diagnosis codes in compliance with federal regulations and agency policies. This role ensures that coding supports medical necessity, aligns with OASIS assessments, Face to Face (F2F), and facilitates timely billing and reimbursement. The coder collaborates closely with clinical and billing teams to maintain documentation integrity and optimize revenue cycle performance.

Key Responsibilities

Coding & Documentation Review

• Analyze clinical documentation to assign appropriate ICD-10 codes for home health episodes.
• Ensure coding re ects accurate primary and secondary diagnoses, comorbidities, and aligns with OASIS assessments.
• Validate that documentation supports medical necessity and meets payer-speci c requirements.
• Collaborate with clinicians to clarify documentation and resolve coding discrepancies.
• Maintain coding compliance with CMS guidelines, including PDGM (Patient-Driven Groupings Model) standards.

Billing & Work ow Integration

• Support billing team by ensuring timely and accurate coding for claim submission.
• Utilize Home Care Home Base (HCHB) and Strategic Health Partners (SHP) to manage coding work ows and documentation.
• Assist in resolving coding-related claim denials and participate in appeals when necessary.

Communication & Quality Assurance

• Communicate proactively with clinical staff regarding documentation and coding needs.
• Participate in internal audits and quality reviews to ensure coding accuracy and regulatory compliance.
• Identify trends in documentation or coding errors and recommend corrective actions.

Compliance & Education

• Stay current with ICD-10 updates, CMS regulations, and industry best practices.
• Maintain HIPAA compliance and con dentiality of patient information.
• Assist in training new staff on coding protocols and documentation standards.

Quali cations

Education & Experience

• High School Diploma or GED required; Associate's or Bachelor's degree in Health Information Management or related eld preferred.
• Certi ed Coding credentials (e.g., CCS, CPC, or HCS-D) required.
• Minimum 2 years of medical coding experience; home health coding experience strongly preferred.
• Experience with Home Care Home Base (HCHB) is required or highly preferred.

• In-depth knowledge of ICD-10 coding, PDGM, and CMS home health regulations.
• Strong analytical skills and attention to detail.
• Ability to work independently and manage multiple priorities.
• Effective communication and collaboration skills.
• Pro ciency in electronic health record systems and coding software.
